We strive to provide as much transparency as possible to enable you to make an informed decision about your recommended treatment. Before proceeding with treatment, you’ll be asked to sign an Informed Treatment Consent form which will cover:
— the condition which is being treated
the recommended surgical procedure (together with any alternatives),
— the outcomes that you will likely experience,
and the potential for complications, side effects or limitation, both during the procedure and the recovery process.
— You’ll be informed of potential complications of any recommended treatment, the need for any additional diagnostic procedures and asked for consent to use clinical images for research or educational purposes.
If you feel that you need additional information before consenting to treatment, please contact us and we will arrange for a further consultation with Dr Slater. The procedures performed by Dr Slater are purely elective and it’s entirely YOUR decision to proceed with orthopaedic surgery.
Separate to this treatment consent, you will also be required to sign an Informed Financial Consent (IFC) form which will cover all of the associated costs of your treatment.
What to bring to your first consultation
Please arrive 10 minutes before your appointment time to complete your new patient forms and pain assessment documents.
Please remember to bring the following to your first appointment:
— Current x-rays and scans. If you’re unable to obtain copies of these, please call our office and we’ll help you to source these. Without them an additional appointment may be required to accurately assess your condition.
— A current referral from your GP or specialist. This is necessary for you to receive partial reimbursement from Medicare for your appointment.
— Medicare, DVA, Private Health Insurance or Work Cover details if applicable.
Cost
Part of your initial consultation fee will be eligible for a Medicate rebate. You’ll need to make the full payment, but we’re happy to process the Medicare claim for you.
Support Person
Many people find it helps to bring a support person with them – your partner or a friend. You’re very welcome to bring then with you.
What can I expect?
Dr Slater allocates 30 minutes for your initial consultation. However, each case is different and will depend on your condition. If a surgical procedure is recommended, your appointment may extend to 60 minutes. While he will discuss all of your treatment options, the choice to proceed with a surgical procedure is entirely yours.
We’re committed to ensuring that you understand all associated costs relating to surgical procedures and will ask you to sign an Informed Financial Consent Form before proceeding with any treatment. After your consultation, we will provide you with a fee estimate including any out of pocket expenses.
