INFORMED TREATMENT CONSENT

We strive to provide as much transparency as possible to enable you to make an informed decision about your recommended treatment. Before proceeding with treatment, you’ll be asked to sign an Informed Treatment Consent form which will cover: 

  • the condition which is being treated
  • the recommended surgical procedure (together with any alternatives),
  • the outcomes that you will likely experience,
  • and the potential for complications, side effects or limitation, both during the procedure and the recovery process.

You’ll be informed of potential complications of any recommended treatment, the need for any additional diagnostic procedures and asked for consent to use clinical images for research or educational purposes.  

If you feel that you need additional information before consenting to treatment, please contact us and we will arrange for a further consultation with Dr Slater. The procedures performed by Dr Slater are purely elective and it’s entirely YOUR decision to proceed with orthopaedic surgery.

Separate to this treatment consent, you will also be required to sign an Informed Financial Consent (IFC) form which will cover all of the associated costs of your treatment.
